MHSAA Physical Form – This form needs to be completed after April 15 to be valid for the upcoming school year.
RevTrak – Link used to pay participation fees and administration fees for sports teams.
Athletic Fee Adjustment Form – Complete and return to the Athletic Office. Qualified families may have their Athletic Fee waived or reduced.
Registration Fees:

- High School Athletics: $175

- Middle School Athletics: $100

- Aministrative Fee for Self-Funded Sports teams: $50

- Self-Funded Sports will have additional fees to be paid directly to the team.
